   forwarded to Kay International plc prior to going on cover. Failure to indicate
   any material facts at the quotation stage will jeopardise the terms quoted

* Should you wish to take up the quote given by Kay International plc,
   the online proposal/statement of facts must be signed and sent to
   Kay International plc

* Premium must be settled with Kay International plc within 21 days
   from the date of inception. Failure to do so may jeopardise your policy cover
